Request for Disclosure of Personal Information

- Write us email (info@fukuokasoko.com) or fax (+81-92-621-2373) and type the word "Information Disclosure Request" in the subject line. In response we will send you the prescribed request form.
- 2.
- Please fill out the form and send it by registered mail, enclosing the charge and documents referred in 3, 4 as below.
- 3.
- As for disclosure requests, we charge you the handling cost of 1000 yen. (The charge includes 500yen for the delivery cost from us to respond to you.)
- 4.
- To confirm that the request is made by the guaranty principal or deputy authorized by the principle, we require to provide a copy of public certificate. We will send the requested information only to the address mentioned in the certificate.
Public Certificate
- For the guaranty principal
Submit each of the following documents;
Driver's License, Health Insurance Card, Passport, Basic Resident Register Card or Foreign Resident Registration Card
- For the deputy authorized by the principal
Submit both documents listed below as A and B
A. A letter of proxy with impression of a registered seal and a seal certificate
B. Each of the following documents from both guaranty principal and deputy
Driver's License, Health Insurance Card, Passport, Basic Resident Register Card or Foreign Resident Registration Card
-The Handling of a Copy of Public Certificate Including Personal Information-

- We use personal information collected in the process of handling disclosure request only for the purpose of identity verification of the principal and deputy.
- 2)
- We will not provide or disclose the information to a third party without the consent of the principal, except in any of the following cases.
- 1)
- When it is required by laws
- 2)
- If it is difficult to obtain the principal's consent, but use of the personal information is required to protect human lives, bodies and/or properties.
- 3)
- If it is difficult to obtain the principal's consent, but the use of the personal information is particularly necessary to improve public hygiene or promote the welfare or well-being of children.
- 4)
- If it is necessary to cooperate with national agencies, local governments, or their consigned parties in the execution of legally prescribed business, but where obtaining the principal's consent may hinder execution of the said business.
- 3)
- We will not subcontract the handling of personal information to a business partner.
- 4)
- While the customer provides personal information at his/her own discretion, the customer must understand that he/she may suffer from the disadvantages if he/she decides not to submit such information.
